INFORMED THE PERSON-IN-CHARGE OF THE COMPLAINT THAT WAS FILE WITH THE HEALTH DEPARTMENT.
COMPLAINT INSPECTION WAS CONDUCTED ALONGSIDE THE FACILITY'S ROUTINE INSPECTION (SEE "ORIGINAL INPSECTION" DATED 2/10/2022). DURING THE INSPECTION, NO LIVE PESTS WERE FOUND, HOWEVER EVIDENCE OF DEAD INSECTS WERE OBSERVED. AT THE SALAD PREP STATION, DEAD INSECTS WERE OBSERVED WITH THE OVERHEAD LIGHTS. IN ADDITION, RODENT DROPPINGS WERE FOUND ON THE FLOOR BELOW THE HAND WASHING STATION LOCATED AT THE SERVICE STATION. ALSO, MOUSE DROPPING WERE FOUND IN THE UTILITY ROOM NEAR A HOLE IN THE WALL. THIS FACILITY DOES HAVE A PEST CONTROL MANAGEMENT AGENCY THAT SERVICES THE RESTAURANT TWICE A MONTH--A REVIEW OF THE PEST CONTROL INSPECTION REPORTS CONFIRMS THAT PEST CONTROL ACTIONS ARE BEING TAKEN. THIS FACILITY HAS BEEN PUT ON A CLEANING SCHEDULE TO REMOVE DEAD INSECTS AND RODENT DROPPING. IN ADDITION, THE FACILITY HAS SCHEDULED REPAIRS TO PREVENT PEST MOVEMENT WITHIN THE FACILITY. |
